DEAR SIR,

Your policy, No. 5641, stands suspended on the
Company's books by reason of the non-payment of the
annual premium due June 30, 1890 amounting to
$32.86; but can be resorted according to the Company's
rules printed on the back hereof. We enclose the regular
Physicians certificate, and upon its return, properly
executed, accompanied by a remittance covering the amount due,
the policyt will be restored and the usual renewal receipt forward-
ed, provided the certificate is satisfactory and, if not, your re-
mittance will be returned to you promptly.

There is a dividend of $6.90 which you can
use as fact paymnet of Premium

It is important that life insurance be kept in force at all times,
so that in case of death, the full benefits will be realized.
